Medicare Payment System Reforms: What Do We Know?

Medicare is testing new ways to pay for medical services, emphasizing value rather than volume, and evidence is beginning to build about successes and challenges. This briefing will examine what we know so far about the basic models, savings, quality, the impact on patients and the prospects for replication.

The Affordable Care Act established initiatives to develop and test these new approaches through the Center for Medicare and Medicaid Innovation (CMMI). The demonstration projects include patient-centered medical homes, accountable care organizations (ACOs), bundled payments and independence at home programs. Currently, these programs account for care provided to about 10 million Medicare beneficiaries.

Cristina Boccuti, associate director, Program on Medicare Policy, Kaiser Family Foundation, will discuss evidence to date regarding savings, quality, access and impact on patients.

Marilyn Moon, institute fellow and director, the American Institutes for Research, will discuss the impact of these models on consumers.

Patrick Conway, principal deputy administrator for innovation and quality and chief medical officer, the Centers for Medicare & Medicaid Services, will spotlight models that are working well, along with milestones and challenges.

Eric De Jonge, director of geriatrics, MedStar Washington Hospital Center, will explain the experiences and results of Washington Hospital Center’s Independence at Home initiative, and MedStar’s plan to adopt this program across its health system.

James Garnham, director, contract and payment innovation, University of Rochester Medical Center, will describe Rochester’s experience with bundled payments.

Marilyn Serafini of the Alliance for Health Reform and Tricia Neuman of the Kaiser Family Foundation will introduce the panel and co-moderate.
