10Briefing  Medicare Advantage: Lessons for the Future, 5/4/2009
The Alliance for Health Reform and the Kaiser Family Foundation sponsored a luncheon briefing on the basics of the Medicare Advantage program. Speakers included: Jonathan Blum, Centers for Medicare and Medicaid Services; Marsha Gold, Mathematica Policy Research; and Mark Miller, MedPAC. Ed Howard of the Alliance and Tricia Neuman of the Foundation were co-moderators. 
10Briefing  Medicare 101, 3/16/2009
Medicare covers nearly 45 million beneficiaries, including 38 million seniors and 7 million younger adults with permanent disabilities. The program is expected to cost the federal government approximately $477 billion in 2009, accounting for 13 percent of federal spending and 19 percent of total national health expenditures. Whom does Medicare serve and what services does it cover? What are Medicare Parts A, B, C and D? How is it structured and financed? What drives Medicare’s costs? How does Medicare reimburse providers and hospitals? What future challenges face the program? To address these and related questions, the Alliance for Health Reform and the Kaiser Family Foundation sponsored a March 16 briefing. 
10Briefing  Using Medicare as Part of Coverage Expansion?, 6/6/2008
A new proposal by The Commonwealth Fund suggests, as a major component of moving toward coverage for all, a Medicare-like program that uninsured adults below the age of 65 could purchase voluntarily. This briefing, co-sponsored by the Fund, featured speakers both favoring and skeptical about this approach.  
10Briefing  Medicare Advantage: Whose Cost, Whose Benefit?, 7/16/2007
The portion of Medicare enrollees in Medicare Advantage plans is on the upswing, now estimated at 19 percent of beneficiaries. By 2016, more than one in four beneficiaries may be covered by a Medicare Advantage plan. This briefing covered the costs and benefits of the Medicare Advantage program, and implications of overpayments to Medicare Advantage plans. Among questions discussed: Are the per-beneficiary overpayments (compared to traditional Medicare) worth it? What would happen if these payments were cut? Should the Medicare Advantage program continue to exist at all? Cosponsored by the Kaiser Family Foundation.

The program cost the federal government approximately $375 billion in 2006, accounting for 13 percent of federal spending.  
10Briefing  BRIDGING THE DIVIDE: Medicare's Role in Reducing Racial and Ethnic Disparities, 1/29/2007
A growing body of evidence shows disparities in quality of care among Medicare beneficiaries of different racial and ethnic backgrounds. These disparities are particularly noteworthy in Medicare, which provides nearly universal access to care to the elderly without regard to race or ethnicity.  
10Briefing  Medicare Advantage: Early Views and Trendspotting, 5/19/2006
The Medicare Prescription Drug, Improvement, and Modernization Act (MMA) of 2003 made a number of changes to the Medicare managed care program – Medicare Advantage. Among the aims: Keep managed care plans from leaving the program by offering higher payments, and give beneficiaries more reasons to consider joining. Is the Medicare Advantage program now growing again after a period of declining enrollment? Are additional changes to payments for Medicare managed care plans warranted?  
10Briefing  Making Sense of Medicare's Drug Benefit: Information and Resources to Help Beneficiaries, 11/7/2005
To help understand the basics of the new Medicare prescription drug benefit, the nonpartisan Alliance for Health Reform and the Kaiser Family Foundation held a briefing especially for congressional staff, including district and state staff, but also of interest to others. Panelists were Tricia Neuman of the Kaiser Family Foundation’s Medicare Policy Project; Jack Vogelsong of Pennsylvania’s State Health Insurance and Assistance Program; Beatrice Disman of the New York Region of the Social Security Administration; and Julie Goon of the Centers for Medicare and Medicaid Services.  
10Briefing  MEDICARE BASICS, From (PART) A to D, 5/16/2005
Medicare covers nearly 42 million beneficiaries who are elderly, or have a disability or end stage renal disease. Spending on Medicare benefits accounted for 17 percent of the nation’s total health care spending in 2004. The Medicare Modernization Act of 2003 made many changes to the program including the addition of a prescription drug benefit (“Part D”), which will begin full implementation in 2006. Whom does Medicare serve and what services does it provide? How is the program financed? What are the basics of the new Medicare drug benefit? To help answer these questions and more, the nonpartisan Alliance for Health Reform and the Kaiser Family Foundation held a May 16 luncheon briefing for those new to the topic of Medicare and those who wanted to refresh their knowledge.  
10Briefing  How Good is the Quality of Care in Medicare? , 5/6/2005
The federal government’s responsibility to provide access to health care for the nation’s 41 million Medicare beneficiaries implies another obligation: to spend taxpayer dollars wisely. This means assuring that the $250 billion+ spent for Medicare goes for services that are safe, timely and effective. How well is the Medicare program meeting these goals? Why is there an inverse relationship between how much is spent per Medicare beneficiaries across the states and the quality of care rankings for those states? Why are there unexplained variations in care at the end of life? What is the relationship between staffing levels and quality? What should be done to boost the quality of care for Medicare beneficiaries while assuring the most effective use of resouces?

10ToolkitToolkit: Medicare Private Fee-for-Service Plans, 7/13/2007
Over the past three years, enrollment in Medicare private fee-for-service (PFFS) plans has increased significantly. These plans offer a potentially greater choice of providers than beneficiaries will find in Medicare HMOs or PPOs. They often provide extra benefits not found in traditional Medicare. Beneficiaries attracted to the plans hope to lower their out-of-pocket costs compared to what they would pay in traditional Medicare. However, the plans have drawn the interest of federal budget cutters since they cost more per beneficiary than traditional Medicare. Moreover, beneficiaries have been reporting confusion about the plans and sometimes, enrollment fraud. Some private fee-for-service beneficiaries have been denied services by physicians who previously accepted their traditional Medicare coverage. This toolkit, supported by the Robert Wood Johnson Foundation, contains resources that describe the basics of PFFS plans, advantages and incentives included in the plans, and the challenges that PFFS enrollees have encountered along the way.  (Adobe Acrobat PDF: 51 kb)

This briefing explored the major provider payment initiatives in the new health reform law. Topics discussed included demonstration projects included in the law, the new Center for Medicare and Medicaid Innovation, and the real-world practicalities involved in changing the way providers are paid. Cosponsored by The Commonwealth Fund, a philanthropy based in New York City and Washington, DC.  
9Briefing  Health Legislation 2007-2008: What's Possible?, 12/13/2006
Suddenly, Capitol Hill is talking about health issues again. Incoming House Speaker Nancy Pelosi promises that Democrats will roll back the prohibition against the federal government negotiating drug prices on behalf of Medicare beneficiaries. (A Kaiser Family Foundation poll released today found broad bipartisan support for this idea.) Incoming Senate Majority Leader Harry Reid says one of his three top priorities will be more funding for stem cell research. Others want to revisit the importation of prescription drugs from Canada. Additionally, the 110th Congress will consider whether to continue the State Children’s Health Insurance Program in its present form.

The Medicare Savings Programs provide assistance with premiums — and in some instances, cost sharing requirements — to Medicare beneficiaries of limited income and resources who do not qualify for full Medicaid benefits. What lessons does Part D enrollment offer that could apply to the savings programs? What techniques are working well for the savings programs that could apply to Part D enrollment? How could newly released recommendations from The National Academy of Social Insurance (NASI) improve the Medicare savings programs if implemented? How feasible are these recommendations?

Medicare beneficiaries can start signing up for prescription drug plans on November 15. Already, health plans have announced that they will be spending up to $80 million each to promote their offerings. The federal government and nonprofit groups are well into a massive effort to educate the public. But beneficiaries and those they rely on still have many questions about the program. The Alliance for Health Reform sponsored a briefing on Sept. 27 to help reporters with their coverage of the new benefit's implementation and offer story ideas.

Its ability to align incentives through pay-for-performance will have implications for the health delivery system at large. With baby boomers retiring, and Medicare benefits changing, this percentage is expected to grow in the future. How concerned should we really be with rising health care costs, especially costs connected with the care of older people? Will new technologies create efficiencies that will dramatically reduce costs, or will technologies raise demand for care and thus drive cost increases?

President Bush’s FY 2006 budget could have long-lasting effects on several health programs. If enacted as is, the budget would trim $60 billion in the growth in Medicaid spending over the next 10 years. At the same time, the budget offers $11 billion in new money to enroll children in Medicaid and the State Children’s Health Insurance Program. It would provide $74 billion in tax incentives to help the uninsured buy coverage. Some congressional budget leaders have signaled their intention to find budget savings in another entitlement program – Medicare. After the hearings are over and the votes taken, how many of the president’s health initiatives will remain? What budget proposals originating with Congress might be adopted? How will discretionary programs – like the Centers for Disease Control and Prevention, and the National Institutes of Health – be affected? What are the long-term implications for health care providers and consumers, and for the deficit?

Come January, we can expect to see some new approaches to health legislation. Sen. Harry Reid, incoming Senate majority leader, says one of his top three priorities will be more funding for stem cell research. House Speaker-to-be Nancy Pelosi wants a vote early on to roll back the prohibition against the federal government negotiating prescription drug prices for Medicare beneficiaries. And these are just two of the health issues likely to be debated in the new Congress. The White House too will have a definite voice in the debates to come.
