The following is a list of briefings, documents, and publications that have been ranked by the Alliance for Health Reform in order of relevance, 10 being "Exclusively Related" to the issue, to 5 being "Somewhat Related" to the issue. Almost every day, we learn of a new hearing or briefing about ways to improve our health care system. The White House, Congress, advocates and stakeholders are fully engaged. The elephant in the room during those discussions is long-term care. Despite being so large, it suffers from the possibility of being ignored. This March 9 forum, supported by the SCAN Foundation, brought together opinion leaders and stakeholders to explore options for the future of long-term care in the reform debate. Participants considered key policy questions on how to move from testing models at the edges to implementing feasible options through health reform. 
10Briefing  Changing the Culture and Improving Quality: Innovations in Long-Term Care, 10/19/2007
Culture change in the long-term care world involves many players – residents, administrators, workers, lawmakers, policy analysts – sharing a common vision. One such vision attracting attention has been developed by a group of citizens, providers and advocates known as Pioneers who are exploring alternatives to traditional nursing facilities. Their goal: facilities that are resident-centered, less institutional and more home-like. This involves trying to piece together financing from Medicaid, Medicare and private funding sources. Will we be ready to meet the demands of aging boomers for more consumer-directed care? Are legislative changes needed to promote it? This briefing, sponsored by the Alliance for Health Reform and the Robert Wood Johnson Foundation, addressed these and related questions.  
10Briefing  THE LONG-TERM CARE PARTNERSHIP PROGRAM: What Role Will It Play in Broader Long-Term Care Policy?, 11/9/2006
The Long-Term Care (LTC) Partnership Program allows purchasers of private long-term care insurance to qualify eventually for Medicaid coverage of long-term care services without having to meet the same asset requirements as other Medicaid applicants. Partnership programs began in 1987 in four states – New York, California, Connecticut, and Indiana.  
10Briefing  CARING FOR THE ELDERLY: Is There Any Answer to Rising Health Costs?, 9/26/2005
The costs of caring for the elderly have been growing steadily higher. Spending on Medicare benefits accounted for 17 percent of the nation’s total health care spending in 2004. With baby boomers retiring, and Medicare benefits changing, this percentage is expected to grow in the future. How concerned should we really be with rising health care costs, especially costs connected with the care of older people? Will new technologies create efficiencies that will dramatically reduce costs, or will technologies raise demand for care and thus drive cost increases? 
10Briefing  Cash and Counseling: Part of the Long-Term Care Answer?, 7/29/2005
“Cash and Counseling” is a way of allowing individuals receiving personal assistance services through Medicaid to have more control over the services they get and who provides them. The program provides an individualized allowance that beneficiaries can use to hire a personal care assistant of their choice – often a family caregiver – or purchase items that help them live independently. The Alliance for Health Reform, along with the Robert Wood Johnson Foundation, held a briefing to discuss Cash and Counseling and results from the program's demonstration.

On March 9, 2009, the Alliance for Health Reform held a policy roundtable entitled "Long Term Care: Exploring the Possibilities," co-sponsored by The SCAN Foundation. The white paper produced for the roundtable was authored by Joshua Weiner of RTI International, with review and comments by Sheila Burke of Harvard University. The paper, "Long Term Care: Options in an Era of Health Care Reform" frames the main issues of long-term care financing and delivery and lays out a comprehensive set of reform options. (Adobe Acrobat PDF: 961 kb)

About a third of those turning 65 in 2010 will need nursing home care during their lifetimes. Yet so many people dread the thought of spending their last days or years in a nursing home. This is because many people aren’t aware of the creative alternatives being developed for those needing such care – facilities such as “Green Houses” that are resident-centered, less institutional and more home-like. This issue brief addresses the challenges being faced by those trying to change the nursing home culture, and some of their successes. Some Medicare beneficiaries receive significantly more hospital-based services during the last two years of life than do other beneficiaries. The number of physician visits for Medicare beneficiaries can also vary greatly. What accounts for this variation? Do patients receiving more services tend to get better care, or not? Should policymakers take steps to more closely examine the relationship between spending and the volume of services provided at different facilities?

Seventeen years ago, a bipartisan congressional commission agreed unanimously that “all Americans should have access to affordable health and long-term care coverage in an efficient and effective system.” The Pepper Commission, chaired by Sen. Claude Pepper and then by Sen. Jay Rockefeller, is all but forgotten. But the insights and recommendations contained in the commission’s final report make for fascinating reading today and offer guidance as the nation ramps up for another try at major health reform.
