10Briefing  Quality Care: Getting More Bang for the Buck?, 11/15/2013
Health care policy leaders are counting on public and private initiatives, such as paying for performance, to improve value in the health care equation in which cost and quality at times seem to be at odds. Health care costs are high and quality outcomes are low in several metrics, compared to other developed nations. And, studies have shown that costs vary among states and within states, and that regions with higher costs do not necessarily have higher quality.  
10Briefing  Managing Costs and Improving Care: Team-based Care of the Chronically Ill, 8/11/2011
Treating those with multiple chronic conditions, including the elderly and disabled populations, accounts for 30 percent of total U.S. health care spending as of 2010. Half of this amount is spent by Medicare and Medicaid on behalf of beneficiaries eligible for both programs. It may be possible to improve the quality of care for the chronically ill while altering the trajectory of spending for their care. Savings have been shown in some private and public sector approaches using teams that span multiple sites of care, reduce fragmentation and improve health outcomes. How can Medicare, Medicaid, private plans and providers partner to develop new approaches and achieve these public health goals? This briefing, cosponsored by The Commonwealth Fund, addressed these questions and more.  
10Briefing  Public Reporting of Quality Outcomes: What’s the Best Path Forward?, 4/27/2011
More than 25 years have passed since what is now the Centers for Medicare and Medicaid Services (CMS) began publishing hospital mortality data. Previous efforts suggest that public reporting can add significant value. Yet there are concerns about the best way to measure outcomes and quality, the possible unintended effects of public reporting, and whether purchasers and consumers actually use the information to make choices. What is the exact nature of public reporting? How might it affect patients, purchasers and providers of care? What role can public reporting play in the larger effort to improve the quality and lower the cost of health care? What can we learn from previous efforts? How does the Affordable Care Act affect public reporting? What risks, if any, are involved? This briefing, cosponsored by The Commonwealth Fund, addressed these and other questions.  
10Briefing  Community Coalitions: Pursuing Better Quality Health Care One Locality at a Time , 4/15/2011
Stakeholders in dozens of communities around the nation are taking action to improve quality of care locally by engaging in one or more collaborations. Many are engaged in one or more such initiatives as Aligning Forces for Quality, a Robert Wood Johnson Foundation project; Chartered Value Exchanges, sponsored by the Agency for Health Care Research and Quality (AHRQ); and the Beacon Community Program, run by the Office of the National Coordinator for health information technology. What does each program offer? What goals do they have in common? How do they relate to a national quality strategy? This briefing, cosponsored by the Robert Wood Johnson Foundation, addressed these questions and more. 
10Briefing  Shared Medical Decision Making: We're in This Together , 2/14/2011
The relationship between patients and doctors has been changing from the traditional model in which a doctor decides on a treatment course without significant patient input. One way to facilitate shared decision making is through the use of decision support tools, which offer the patient high quality medical information in an easily understandable format. How do these tools work? How might shared decision making benefit patients, purchasers and providers of care? What role can shared decision making play in the larger effort to improve the quality and lower the cost of health care? How does the Affordable Care Act affect shared decision making? To address these and related questions, the Alliance for Health Reform, the National Committee for Quality Assurance and the Foundation for Informed Medical Decision Making sponsored a February 14 briefing. 
10Briefing  Rural Health: Laying the Foundation for Health Reform, 10/30/2009
Health care delivery poses unique challenges in rural communities. Provider shortages are particularly acute in rural America, where levels of educational achievement are generally lower and there are fewer opportunities to receive medical training. Unemployment and uninsurance rates are higher in rural areas than elsewhere. What provisions in the various reform proposals affect rural health care? What particular challenges need to be overcome in order to improve care delivery in rural areas? What aspects of health reform will require special accommodation in rural areas? Are delivery systems in rural areas currently able to handle the influx of new patients coverage expansion would bring? How does HIT factor into rural reform? To answer these questions and more, the Alliance for Health Reform hosted a briefing with the Robert Wood Johnson Foundation on October 30.

As the health reform debate heats up in summertime Washington, one key consideration is how to provide better value for the health care dollars spent. This briefing, cosponsored by the United Health Foundation, addressed questions such as: How can the private sector contribute to a slowing of national health care expenditures? Are there examples of models that have been implemented in the marketplace and have demonstrated they can save money? 10Briefing  AIMING HIGHER: Results from a State Scorecard on Health System Performance , 6/15/2007
Following up on last year’s national scorecard on the U.S. health system performance, The Commonwealth Fund has developed a state scorecard to assess state variations in key areas of access, quality, hospital use and cost, equity and health outcomes. Results of the state-level comparisons were reviewed and analyzed at a luncheon briefing sponsored by the Fund and the Alliance for Health Reform.

There is mounting research highlighting poor quality of care and inefficient spending in our health care system. At the same time, diagnostic and treatment options proliferate at an accelerating pace.  
10Briefing  SCHIP and BEYOND: Improving Health Care Coverage and Quality for Children, 3/12/2007
Dollar for dollar, providing better health care for children represents one of the best returns on investment available in the U.S. This involves not only better monitoring of quality and making quality improvements, but also enrolling eligible children in health programs such as SCHIP and Medicaid. And it means ensuring that health care systems – including safety net providers and health insurers – are responsive to the unique health needs of children.  
10Briefing  Pay-for Performance and Medicare: Moving from the Drawing Board to the Doctor's Office, 12/15/2006
As the nation’s largest payer for health care services, the Medicare program has a major effect on payments to providers. Its ability to align incentives through pay-for-performance will have implications for the health delivery system at large. A recent Institute of Medicine report made recommendations for implementation of “P4P” under Medicare.  
10Briefing  Measuring up: A Comprehensive Scorecard for America's Health System, 10/11/2006
A new assessment by The Commonwealth Fund finds that deficiencies in the nation’s health care system cost 100,000 to 150,000 lives and $50 to $100 billion annually. The scorecard compares national averages for the U.S. to national and international benchmarks for 37 indicators. The U.S. received a composite score of 66 out of 100. On one quality measure, mortality, the U.S. ranked 14th out of 19 countries.

Starting in 2014, employers will be allowed to charge their workers up to 30 percent more for health insurance premiums if they don't meet certain health goals, under the Patient Protection and Affordable Care Act (ACA). An Alliance for Health Reform briefing, Worker Wellness Programs: Do They Work? explained the provisions in the law, and examined employer efforts to improve worker wellness, along with evidence about savings. What effect can tying wellness incentives to premiums have on workers, and especially less healthy workers? What does the evidence say about wellness programs and costs? Under the proposed agency rules related to the ACA provisions, what would be permitted and not permitted in wellness program design and practice? What are key concerns about implementing such programs? A distinguished panel of experts addressed these and related questions. 
9Briefing  The Alphabet Soup of Care Delivery Transformation, 9/10/2012
There is widespread agreement that the current health care delivery system is fragmented. Your primary care physician may be the last to know what your cardiologist is doing, or your radiologist or pharmacist, for that matter. However, change is in the works. Both the private and public sectors are testing various care delivery transformation models to improve quality, reduce morbidity and mortality, and contain the costs of treatment. The alphabet soup includes Accountable Care Organizations (ACOs), Patient Centered Medical Homes (PCMHs) and other models. What do these acronyms signify? Which care coordination models is CMS supporting? Has the Innovation Center found any promising practices that may be scalable? Have these models shown a reduction in spending? Improvements in quality of care? How is improvement being measured? Health care access, particularly access to primary care, is on the minds of all who are following health reform efforts in Congress. 9Briefing  Who Cares What Patients Think?, 12/4/2006
The growing complexity of our health care system, and time demands on providers, can cause patients’ non-obvious needs to get lost in the shuffle. Sometimes, the result can be less-than-optimal care. In response, providers are trying to be more responsive to patients’ cultural traditions, family situations, personal preferences and values. Some have tagged this movement “patient-centered care.”

According to a recent survey, a number of states have expressed interest in expanding managed care approaches within their Medicaid programs. But while managed care may present an opportunity for better delivery of care, it presents challenges within certain populations and geographic areas. What are the different models for managed care within the Medicaid program? What impact would these changes have on beneficiaries, providers and health plans? What have we learned from past efforts to expand managed care? How are states planning to use managed care to prepare for 2014, when 16 million newly qualified beneficiaries begin enrolling in Medicaid as authorized by the Patient Protection and Affordable Care Act? In addition to expanding coverage to 32 million previously uninsured Americans, the health reform law of 2010 aims to improve the health of the population and the quality of health care delivery, while reducing costs. One tool to pursue these goals is the creation of a national voluntary program for accountable care organizations (ACOs). The ACO provision and the proposed rule raise many questions. How can health care providers organize to provide more patient-centered, coordinated, accountable care? How will the new program affect the relationships among primary care providers, specialists, and hospitals? Will coordination among providers mean concentration of market power and higher prices? What lessons can be learned from existing ACOs about achieving better care, better health, and lower costs? To address these and related questions, the Alliance for Health Reform and The Commonwealth Fund sponsored a May 13 luncheon briefing.

Now that health reform is being discussed in earnest on Capitol Hill, some wonder if we will be able to maintain our existing choices under a reformed health care system. Almost every day, we learn of a new hearing or briefing about ways to improve our health care system. The White House, Congress, advocates and stakeholders are fully engaged. The elephant in the room during those discussions is long-term care. Despite being so large, it suffers from the possibility of being ignored. This March 9 forum, supported by the SCAN Foundation, brought together opinion leaders and stakeholders to explore options for the future of long-term care in the reform debate. Participants considered key policy questions on how to move from testing models at the edges to implementing feasible options through health reform.

Chronic diseases are the number one cause of death and disability in the United States and account for three quarters of the nation’s health spending. There is mounting evidence that the U.S. system is often financially inefficient and delivers poor quality. There is also research suggesting that having so many people without insurance can adversely affect the quality of care for everyone, even those with insurance. A new report by The Commonwealth Fund sets forth principles for providing universal coverage in ways that could promote a high performance health care system. Several states are already trying to reform their health care systems. How is implementation of universal coverage progressing in Massachusetts? How are negotiations moving forward in California and Pennsylvania to pass health care reform legislation? In addition to improving access and quality, what else should states do to improve the health and health care of their citizens? This briefing, sponsored by The Commonwealth Fund and the Alliance, addressed these and other important questions.

Come January, we can expect to see some new approaches to health legislation. Sen. Harry Reid, incoming Senate majority leader, says one of his top three priorities will be more funding for stem cell research. House Speaker-to-be Nancy Pelosi wants a vote early on to roll back the prohibition against the federal government negotiating prescription drug prices for Medicare beneficiaries. And these are just two of the health issues likely to be debated in the new Congress. The White House too will have a definite voice in the debates to come.

A new Alliance for Health Reform video features Princeton’s Uwe Reinhardt questioning whether bundling payments for medical services might actually lead to higher – not lower – costs.

"The ACO's, the accountable care organizations, could create local monopolies that could dictate to you what that bundled price would be, and some of us fear that bundled prices might be even more than what the fee-for-service for that bundle would be today. … You really should align all the payers and say, 'Let us jointly negotiate with the ACOs what those bundles should be so that they cannot divide and rule and sort of make us on the buy side weak.'"

Sister Carol Keehan on Health Law Enrollment Challenges this Year

Jonathan Blum on CMS Efforts to Keep Medicare Spending Growth Down

Jonathan Blum, acting principal deputy administrator and director of the Center for Medicare at the Centers for Medicare and Medicaid Services (CMS), addresses the slower growth of Medicare spending over the last few years, and what his agency is doing to try and continue the trend. “There are promising signs that this strategy to change the payment system, to change the payment models, to focus on waste and abuse, is paying off," he said. "We are taking a whole new approach to addressing fraud in the program. Much more data resources, much more on the ground reaction. We have seen dramatic spending declines in areas of spending, such as home health and durable medical supplies that historically fueled lots of the fraud.”  FULL TRANSCRIPT

Two Former Medicare Administrators Discuss How to Save the Program


A new Alliance for Health Reform video features two former Medicare administrators -- Gail Wilensky and Bruce Vladeck -- on their ideas about how to save the program.

Ms. Wilensky, who ran Medicare in a Republican administration, argued for increasing the eligibility age of Medicare for future retirees, while still making the program available at age 65 for those who are disabled.

Douglas Holtz-Eakin: Health Care Spending Lull Only Temporary

A new Alliance for Health Reform video features Douglas Holtz-Eakin of the American Action Forum, and a former director of the Congressional Budget Office, arguing that recent slower spending growth in health care won’t continue.

"We also saw a slowdown in the mid-90s, and we all declared victory and it came right back. I think next year we’ll see a noticeable uptick. There will be lots of new people entering the insurance markets because of the exchanges and the subsidies that come along with them, and those subsidies are very generous. … These are an invitation for people to get coverage and to buy more health care. I think that’ll place a lot of pressure on spending."

A new Alliance for Health Reform Toolkit summarizes the recent policy debate surrounding the decade-long problem In the last six months, the House Energy and Commerce Committee, House Ways and Means, and Senate Finance committees all put forth similar bills to repeal and reform the flawed Medicare physician payment formula. Further, with the CBO estimating the cost of repeal at $116.5, the lowest since 2006, many health policy experts are speculating that 2014 is the year that policymakers enact a permanent change to the SGR.
